Resumo: A 56-year-old male patient with no associated comorbidities presented with a painless nodule in the right cervical and parotid regions, accompanied by otalgia, which had been going on for 1.5 years. Physical examination revealed a soft, mobile nodule in the right parotid region, close to the lobe, measuring around 3 cm, mobile, painless to the touch, with no lymph node enlargement. Computed tomography revealed a nodular thickening in the anterior region of the superficial lobe of the right parotid gland, measuring around 15 x 12 x 12 mm. Fine needle aspiration showed a solid, septated lesion, while microscopic analysis suggested a basaloid neoplasm. The patient underwent right parotidectomy, with neoplasm-free margins. The definitive diagnosis was intercalated duct adenoma, with no signs of malignancy. No additional therapies were required. The patient remained well under close follow-up and showed no evidence of disease 10 months after surgery.
