Resumo: A 39-year-old man presented with a progressively enlarging exophytic lesion in mandibular region, measuring approximately 10 cm. Computed tomography revealed an expansive mass involving the body and ramus of the right mandible, with extension into adjacent structures. Histopathological analysis identified a malignant, infiltrative small cell mesenchymal neoplasm. Immunohistochemical evaluation demonstrated FLI1 positivity, suggestive of Ewing sarcoma. Chemotherapy was administered, without therapeutic response. Subsequently, the patient underwent a right segmental mandibulectomy with microsurgical reconstruction using a fibular free flap. Histopathological analysis of the resected specimen revealed osteosarcoma with a heterogeneous pattern, including osteoblastic/sclerosing and pleomorphic variants, as well as a focal area with round cell morphology. Postoperatively, the patient developed complications at the graft site, including exposure of the fixation plate, necessitating a new surgical intervention. Five months later, local recurrence and pulmonary metastasis developed. The current management consists of palliative chemotherapy and close monitoring of both local and metastatic lesions.
