Resumo: Peripheral ameloblastoma, a rare, benign, and non-aggressive odontogenic neoplasm, manifests as an exophytic lesion in the gingiva. A 51-year-old man, hemophiliac A, and type I diabetic, complaining of a gingival protuberance, presented with a whitish, smooth, well-defined, and sessile papule on the lingual gingiva of lower left second premolar, measuring 3 mm, painless, and present for 8 months. There were no changes in the periapical radiograph or in the sensitivity tests of both lower premolars. After excisional biopsy, histopathological examination revealed connective tissue permeated by islets/cords of cubic or columnar cells, arranged in a palisade on the periphery of the islets, with inverted polarization and no atypia. Also, they connected focally with the lining epithelium. Immunohistochemical analysis demonstrated positivity for CKs 5/6 and 19, confirming the diagnosis. The patient is well, without recurrence. Due to his discreet clinical presentation, the diagnosis of peripheral ameloblastoma depends on careful clinicopathological correlation.
