Resumo: This report describes two cases of odontogenic keratocysts (OKC) that occurred in the midline. The first case, a 57-year-old male, presented a hypodense image between the roots of the maxillary central incisors, causing displacement of teeth. Under the diagnosis hypothesis of nasopalatine duct cyst, the lesion was excised. The second case, a 33-year-old male reported mobility of uninjured mandibular central incisors. Computed tomography revealed well circumscribed hypodense lesion between the roots of these teeth. An odontogenic cyst or tumor were the main diagnosis hypotheses. The lesion was excised. Microscopically, both lesions appeared as a cystic cavity lined by corrugated parakeratinized squamous epithelium, with basal layer showing columnar cells in palisading arrangement, confirming the diagnosis of OKC. Both patients are under periodic follow-up, with no signs of recurrence after 6 and 72 months of treatment. OKC should be considered in the differential diagnosis of midline hypodense images.
