Título: Palatal pleomorphic adenoma with intravascular emboli in a pediatric patient: a rare presentation

Resumo: A 14-year-old girl presented with a painless swelling on the left side of the palate for six months. Imaging revealed a 22×20×14 mm mass with palatine bone resorption and compression of the left nasal choana. An incisional biopsy was performed due to suspicion of a salivary gland neoplasm. Histopathological analysis revealed a highly cellular proliferation of epithelial and myoepithelial cells arranged in sheets, cords, and duct-like structures interspersed with myxoid, chondroid, and dense connective tissue, with sparse hyalinized areas, consistent with pleomorphic adenoma. Excisional surgery under general anesthesia confirmed the diagnosis, with clear margins. Notably, one neoplastic embolus was identified within blood vessels, despite no other signs of malignancy. The patient remains disease-free after six months of follow-up. This case highlights the rare occurrence of vascular invasion in pleomorphic adenoma and underscores the importance of thorough histopathological evaluation and long-term monitoring, even in histologically benign tumors.
