Resumo: Acute lymphoblastic leukemia (ALL) is the most common childhood cancer. Chemotherapy-induced immunosuppression often leads to oral complications, including opportunistic infections that can delay treatment and affect quality of life. A 13-year-old male patient undergoing treatment for acute lymphoblastic leukemia developed ulcerations on the lower lip. Clinical examination revealed neutropenia (1,123 leukocytes/mm³; 125 segmented neutrophils/mm³) and multiple deep ulcerations on the lower lip and chin. These ulcerations were irregular in shape, presented with purulent discharge for five days, and appeared after the completion of a chemotherapy cycle (MTX 2 g/m²/day; leucovorin 12 mg/m²). Based on the clinical features, an opportunistic infection was the proposed diagnosis, and photodynamic therapy was initiated daily using methylene blue and a red laser at 2 J/cm². A culture of the lip lesions was requested and later revealed the presence of Prevotella intermedia. After 20 sessions, the lesions were fully healed.
