Resumo: A 27-year-old Black female patient, 10 days postpartum and breastfeeding, sought emergency care due to painful oral ulcers causing significant discomfort and difficulty eating. She had been using anticoagulant, and amoxicillin and ibuprofen for three days to treat pericoronitis in teeth 38/48. Clinical examination revealed multiple ulcerated lesions throughout the oral mucosa. The diagnosis was primary herpetic gingivostomatitis. Treatment was initiated with valacyclovir, metronidazole, and antimicrobial photodynamic therapy (aPDT). A 0.01% methylene blue solution was applied for 5 minutes as a photosensitizer, followed by red low-level laser therapy (660 nm, 100 mW, 6 J/point, 60 s/point). By the third session, the patient was asymptomatic, and photobiomodulation therapy was adopted using the same laser (1 J/point, 10 s/point). The clinical response was excellent, with rapid symptom remission and lesion healing. The combination of pharmacological treatment and laser therapy proved highly effective in controlling infection, relieving pain, and promoting tissue repair.
