Resumo: Ameloblastomas are benign but locally aggressive odontogenic tumors that may mimic residual or inflammatory cysts on imaging. A 68-year-old female presented with an asymptomatic unilocular radiolucency in the posterior mandible, initially interpreted as a residual cyst. Surgical enucleation was performed, and histopathological examination confirmed an ameloblastoma, showing epithelial islands with peripheral palisading, reverse polarity, and central squamous metaplasia. The patient was monitored clinically and radiographically over a five-year period with no signs of recurrence. Bone regeneration was satisfactory, and functional integrity was preserved. This case highlights the importance of biopsy and histopathological evaluation in all suspected odontogenic lesions, even when radiographic features suggest a benign process. Early and accurate diagnosis enabled effective conservative surgical management. The successful long-term follow-up reinforces the importance of surveillance, particularly in older patients, and illustrates that lesions with low proliferative index can still require rigorous treatment and monitoring.
