Resumo: Objective: To map shifts in the tissue proteome during malignant transformation of oral leukoplakias (OL) into oral squamous cell carcinoma (OSCC) at spatial single-cell level. Study Design: We collected FFPE tissue from 12 patients with OL, in which 9 progressed to OSCC and 3 did not progress. We performed imaging mass cytometry using one multiplex cocktail containing 34 antibodies to map immune cells (CD45, CD3, CD20, CD19, CD4, CD8a, CD45RO, CD56, FoxP3, Granzyme B, Arginase-1, CD68, CD206, CD163, CD11c, HLD-DR), structural tissue (Pan-CK, α-SMA, CD31, CD44, Vimentin, FAP, ALDHA1), and the functional tissue state (Ki67, PDL1, TIM3, VISTA, CD39, Siglec-15, B7H4, iNOS, CD73, BCL2, TIGIT). Data analysis was performed using machine learning approaches (Steinbock, Python, and R software). Results: We identified 244,948 single cells with distinct expression profiles across patients. High infiltration of immune cells was observed in both OL and OSCC tissues, although immune checkpoint expression was limited. Spatial and immunological differences were detected in the tissue of progressors and non-progressors. Conclusion: This study provides insights into spatial alterations during malignant transformation of OL into OSCC, which has potential for patient risk stratification and future therapeutic opportunities.
