Resumo: Bone-predominant non-Hodgkin lymphoma (NHL) is a rare and aggressive disease, and its differential diagnosis with other osteolytic conditions is often challenging. This report presents a case of NHL with mandibular involvement. A 57-year-old female patient was admitted with bone pain, edema in the left mandibular region, and hypercalcemia. Imaging revealed an expansile, osteolytic lesion in the left mandible (measuring approximately 3 cm), pathological fractures in the femur, and widespread skeletal involvement on PET-CT. Mandibular biopsy revealed large lymphoid cells with scant cytoplasm, granular chromatin, presence of 2-3 nucleoli, Ki-67 (90%), CD20+, MUMI+, BCL2+ and 6+, and C-MYC+. With the diagnosis of NHL-NE, it was completed VI R-CHOP. PET-CT scan of the partial restaging showed significant tumor reduction. Unfortunately, she passed away of unknown causes before final staging. This case highlights the crucial role of hospital-based dental professionals in the early detection and diagnosis of systemic malignancies with oral manifestations.
