Resumo: Odontogenic cysts are common lesions in pediatric oral pathology and often require differential diagnosis due to overlapping clinical and radiographic features. This case reports a 7-year-old male patient (L.G.L.S) presenting a carious lesion on tooth 75 and the radiographic evidence of a lesion in the furcation and alveolar region, initially suggestive of a dentigerous cyst. Excisional biopsy was performed, and macroscopic analysis revealed three darkened, rubbery soft tissue fragments. Histological examination showed connective tissue with areas of chronic inflammatory infiltrate, abscess formation, multinucleated giant cells, and sulcular epithelium sites. The final diagnosis was a nonspecific inflammatory process with abscess areas. This case has been under follow-up for six months. This case highlights the importance of histopathological analysis in establishing accurate diagnoses, especially in pediatric patients where clinical and radiographic findings may be misleading due to similar characteristics of inflammatory and developmental lesions.
