Resumo: Coccidioidomycosis is a systemic fungal infection caused by Coccidioides spp., endemic to arid regions of the Americas. While it primarily affects the lungs, extrapulmonary manifestations, including oral lesions, may occur. A 59-year-old white male, diabetic, with a history of smoking and alcohol use, presented with a painful, resilient white plaque with a rough surface on the left buccal mucosa, with a one-year evolution. Additional fibrotic and edematous lesions were noted on the upper and lower labial mucosa. The initial hypothesis was paracoccidioidomycosis. An incisional biopsy of the buccal mucosa revealed stratified squamous epithelium with pseudoepitheliomatous hyperplasia and exocytosis. The underlying connective tissue exhibited granulomatous inflammation with large, thick-walled, eosinophilic yeast cells, some containing endospores. The inflammatory process extended into skeletal muscle fibers. Histopathological findings were consistent with Coccidioides spp., confirming a diagnosis of oral coccidioidomycosis. The patient was referred to a specialized medical service for further investigation and appropriate management.
