Resumo: Squamous cell carcinoma with a high morbidity and mortality rate when diagnosed late. In this scenario, dental school clinics play an essential role in the early detection of these lesions, thereby improving patient prognosis. Student involvement in this process benefits both academic training and public health by providing care to the low-income population. A 60-year-old Caucasian patient was referred with a lesion on the floor of the mouth suggestive of a neoplasm. An incisional biopsy was performed, and the fragment had a blackish and whitish color, an irregular surface, and a fibrous consistency. Histopathological analysis showed parakeratinized epithelium, cellular atypia, invasion of the lamina propria by islands of pleomorphic neoplastic cells, hyperchromatism and pyknosis. The connective tissue showed intense lymphoplasmocytic inflammation, vessels of different calibers, nerve bundles and glandular structures. The diagnosis, confirmed by immunohistochemistry, was squamous cell carcinoma, emphasizing the importance of biopsy and histologic analysis in early diagnosis.
