Resumo: A 16-year-old male patient presented to the clinic complaining of tingling in the anterior region of the mandible. Clinical examination revealed facial asymmetry on the right side with increased volume in the mandibular body and midline deviation on mouth opening. Intraorally, a slight increase in volume was noted between elements 42 and 44. The panoramic radiograph showed a circumscribed radiolucent area in the same region, associated with the absence of tooth 43. Cone-beam computed tomography (CBCT) allowed visualisation of element 43 and confirmed the presence of a well-defined bone lesion with expansion of the buccal cortical bone. The histological findings showed an odontogenic neoplasm, composed of neoplastic cells of cuboidal to fusiform morphology, organised in a spiral pattern, in interconnected sheets and nests, with reverse polarisation of nuclei, presence of intercellular eosinophilic material and a fibrous capsule delimiting the lesion. The patients prognosis was favourable after complete surgical excision.
