Resumo: A 73-year-old male patient presented a painful submucous nodule in the upper lip with approximately 10 years of duration. Clinically, the lesion appeared as a sessile, painful, and yellowish nodule, presenting firm consistency, rough surface, and measured 2.5 cm of extension. The main diagnostic hypothesis was a squamous cell carcinoma. Under local anesthesia, an incisional biopsy was performed. Histopathological analysis revealed the presence of basaloid neoplastic cells with dark nuclei, arranged in cribriform and tubular patterns. Perineural invasion was observed. According to these features, the diagnosis of adenoid cystic carcinoma was reached. The tumor was staged as T3N0MO. The patient was submitted a wide surgical resection, without intercurrences. Currently, after six months of treatment, the patient is under clinical follow up. Although the adenoid cystic carcinoma is uncommon in upper lip, it should be considered in the differential diagnosis of submucous nodule in this site.
