Título: UNUSUAL MACROCYSTIC VARIANT OF SECRETORY CARCINOMA: A POTENTIAL DIAGNOSTIC PITFALL AMONG BENIGN CYSTIC SALIVARY LESIONS

Resumo: A 35-year-old male patient attended the head and neck surgery ambulatory reporting a nodule in the parotid region that had been progressively growing for 2 years. The CT scan showed an expansive heterogeneous formation in the left parotid gland, measuring around 2.8x2.1x4.4cm. Ultrasound-guided fine-needle aspiration was performed and revealed a benign-looking cystic lesion, suggestive of Warthins tumor. A superficial parotidectomy was performed, from which the pathological evaluation revealed a macrocystic lesion, lined by an epithelium of variable thickness, with areas of mural and luminal growth consisting mainly of papillary projections, with focal appocrine differentiation. S100 and mammaglobin strong positivity confirmed an unconventional presentation of secretory carcinoma. Due to marginal involvement, the oncological team opted for a total parotidectomy and left cervical level Va dissection and facial nerve reconstruction with sural nerve graft. After a 15-month follow-up, the patient shows no signs of local or distant recurrence.
