Título: MANDIBULAR ANEURYSMAL BONE CYST: CASE REPORT AND SCLEROTHERAPY APPROACH

Resumo: A 12-year-old female was referred for evaluation of a left retromolar osteolytic lesion associated with developing tooth 38. Computed tomography revealed a well-defined lesion (21×18×15 mm) without cortical expansion. Surgery revealed an empty cavity suggestive of a simple bone cyst. Two years later, follow-up tomography showed recurrence. A second procedure revealed an empty cavity with vascularized soft tissue distal to tooth 38. Histopathological evaluation revealed blood-filled spaces within connective tissue stroma containing fibrous tissue, extravasated blood, and multinucleated giant cells attached to adjacent bone, confirming the diagnosis of aneurysmal bone cyst. Sclerotherapy with absolute alcohol was performed, resulting in mild mucosal dehydration without significant adverse effects. Post-treatment tomography showed more than 90% bone fill. Aneurysmal bone cyst is a rare, expansive pseudocystic lesion of the mandible, characterized by blood-filled spaces lacking epithelial lining. This case reinforces the importance of differential diagnosis and supports conservative management in recurrent mandibular bone lesions.
