Resumo: A 7-year-old male patient presented with painless swelling in the right preauricular region. Computed tomography revealed a well-defined, expansive lesion (20×20×20 mm) located between the sigmoid notch and the condylar neck. Exploratory surgery identified an empty bone cavity containing soft tissue fragments. Microscopic examination revealed granulation tissue with abundant cholesterol clefts, hemosiderin pigment, foamy macrophages, and multinucleated giant cells, confirming the diagnosis of cholesterol granuloma. A decompression device was installed and maintained for 30 days. Follow-up tomography showed initial bone regeneration. The patient remains asymptomatic with no signs of recurrence. Cholesterol granuloma is a rare reactive lesion of the maxillofacial bones, typically associated with pre-existing bone cavities and chronic inflammatory processes. Histologically, it presents cleft-like cholesterol crystal spaces surrounded by inflammatory cells. This case highlights the importance of including cholesterol granuloma in the differential diagnosis of mandibular intraosseous lesions and supports conservative management as an effective therapeutic strategy.
