Resumo: An 81-year-old Caucasian female patient presented to the general dentist with a chief complaint of a "wound on the tongue" that had appeared approximately three months earlier. She was referred by her dentist after an incisional biopsy suggestive of papilloma. However, due to suspected malignancy she was referred to the Oral Diagnosis for further evaluation. The patient denied tobacco or alcohol use. Extraoral examination revealed no significant findings. Intraorally, a nodular lesion with a verrucous surface, associated with erythematous areas and white plaques, was observed on the dorsal and left lateral border of the tongue. With the main clinical hypothesis of squamous cell carcinoma (SCC), a second incisional biopsy was performed, and histopathological analysis revealed candidiasis associated with areas of epithelial dysplasia. Surgical excisional was then conducted, and the final diagnosis was candidiasis with moderate epithelial dysplasia. The patient is currently under postoperative follow-up in the oral medicine clinic.
