Resumo: This report describes a case of multicentric peripheral ossifying fibroma (FOP) in a 35-year-old male patient in good general health and a non-smoker. Clinical examination revealed satisfactory oral hygiene and multiple soft, asymptomatic nodules in the anterior and posterior gingival regions of both the mandible and maxilla, present for approximately 30 days and prone to bleeding upon stimulation. Laboratory tests showed abnormalities in the leukogram, cholesterol, triglycerides, and uric acid, while serologies for HIV and syphilis were negative. Panoramic and periapical radiographs appeared normal. Excisional biopsies were performed on the anterior lesions and the lesion adjacent to teeth 16 and 17, along with biofilm control measures. Histopathological examination confirmed the diagnosis of FOP. The multifocal presentation of FOP is rare and may be associated with genetic polymorphisms predisposing to gingival overgrowth. The patient remains under biweekly follow-up, with a new biopsy planned due to recurrence in the anterior maxillary region.
