Resumo: Nasopalatine duct cysts are the most common non-odontogenic cysts of the maxilla and often closely mimic radicular cysts on conventional radiographs. Accurate imaging is essential for proper diagnosis and treatment planning. This report describes a 56-year-old male referred for evaluation of a mild swelling of the incisive papilla with palatal secretion. Cone-beam computed tomography (CBCT) revealed a well-defined, unilocular, hypodense, partially corticated lesion located within the incisive canal, measuring 13 mm and containing internal air bubbles. The main radiologic hypothesis was a nasopalatine duct cyst. The lesion was surgically excised, and histopathology confirmed the diagnosis. CBCT was essential in differentiating this lesion from a radicular cyst and provided detailed anatomical information for surgical planning, highlighting its value in evaluating midline maxillary radiolucencies with overlapping features.
