Resumo: A 38-year-old female patient presented with multiple erythematous, symptomatic vesiculobullous and ulcerated lesions located on the right hard palate (teeth 16 and 17) for 3 months. Extraoral examination revealed multiple skin lesions on the trunk, limbs, and head and neck regions. The clinical hypothesis was an autoimmune disease. An incisional perilesional biopsy was performed, showing hyperplastic orthokeratinized stratified squamous epithelium with areas of epithelial disorganization, suprabasal clefting, and acantholytic cells. The underlying fibrous connective tissue exhibited papillary structures and moderate mononuclear inflammatory infiltrate. The final diagnosis was Pemphigus Vulgaris. The patient was prescribed 0.5 mg of oral prednisone and dexamethasone elixir for fifteen days. Clinical evaluation combined with microscopic findings is essential for the final diagnosis. After one year of treatment and follow-up, no recurrence was observed.
