Resumo: A 70-year-old male patient presented with a nodular leukoplakic lesion with a verrucous surface, located on the right labial commissure. The lesion was asymptomatic, with an unknown duration, and the patient had a 45-year history of smoking. The clinical hypothesis was verrucous carcinoma. An incisional biopsy was performed, revealing hyperparakeratinized dysplastic stratified squamous epithelium with exophytic and endophytic proliferation and keratin plugs. The epithelial ridges were thick and invasively compressive, with atypical cells showing binucleation, dyskeratosis, nuclear pleomorphism, and pale cytoplasm. A subepithelial lymphocytic inflammatory infiltrate was also observed. The final diagnosis was verrucous carcinoma, and the patient was referred to an oncology center for surgical excision. Given its indolent nature, dentists should carefully evaluate the clinical and microscopic features for an accurate diagnosis.
