Resumo: A 41yo male presented with a large necrotic swelling, in the mid-portion of the hard palate of 9 months duration. The lesion had a foul odor and the patient reported otalgia and ear fullness with worsening associated with dysphagia and palatal edema. An infiltrative and heterogeneous lesion with poorly defined margins affecting the left masticatory and parapharyngeal spaces, and the ipsilateral maxillary sinus, infiltrating the pterygoid muscles and maxillary sinus, extending into the nasal cavity, invading the soft palate, and the right lateral and posterior walls of the nasopharynx, reaching the right lateral wall of the oropharynx resulting in extensive lysis of the pterygoid plates, the palatine process of the maxilla, and the walls of the right maxillary sinus. The final diagnosis was salivary duct carcinoma rich in osteoclast-like giant cells. The patient was submitted to right infrastructural maxillectomy plus adjuvant radiotherapy and he remains alive free of recurrence.
