Resumo: The study reports the case of a 46-year-old female patient who sought dental care due to a slow-growing, initially painless facial swelling. During the extraoral clinical examination, a volumetric increase was observed in the left midfacial region, associated with exophthalmos. Subsequently, a computed tomography (CT) scan was requested, which revealed a hypodense, expansive, and destructive lesion involving the left maxillary sinus, with invasion of the anterior wall of the maxilla and the orbital floor. An incisional biopsy was then performed, and histopathological analysis demonstrated the formation of tubules and ducts, indicative of a salivary gland neoplasm. The neoplastic cells exhibited basaloid, hyperchromatic nuclei and scant cytoplasm, consistent with a diagnosis of adenoid cystic carcinoma. The patient was subsequently referred for oncologic surgery and underwent a hemimaxillectomy, followed by 33 sessions of radiotherapy, totaling 71 Gy. After 18 months of follow-up, the patient remains free of disease recurrence.
