Título: SYSTEMIC AMYLOIDOSIS WITH ORAL MANIFESTATIONS: A CASE REPORT

Resumo: A 66-year-old male with chronic alcoholism presented with systemic amyloidosis, evidenced by multiple firm, waxy nodular lesions on the labial and buccal mucosa, tongue, floor of the mouth, and periocular region, following a cerebrovascular accident. Laboratory tests revealed renal dysfunction (urea: 148 mg/dL) and elevated C-reactive protein (5.60 mg/dL). Incisional biopsy of oral tissue showed amorphous, lumpy, metachromatic deposits consistent with amyloid, confirming systemic involvement. Clinical signs, especially macroglossia, and widespread oral nodules, indicated advanced amyloid deposition. Differential diagnoses included AA amyloidosis, possibly secondary to hepatic or pancreatic inflammation, and AL amyloidosis, related to plasma cell dyscrasias. This case underscores the diagnostic significance of oral manifestations in systemic amyloidosis. Prompt histopathological confirmation and comprehensive systemic evaluation are essential for early intervention. Management requires a multidisciplinary approach, including treatment of the underlying condition, renal function support, and surgical excision of symptomatic oral lesions to improve both prognosis and quality of life.
