Título: EFFICACY OF L-PRF IN ORAL SURGERY FOR PATIENTS ON BISPHOSPHONATE THERAPY: CASE-CONTROL STUDY

Resumo: Medication-related osteonecrosis of the jaws (MRONJ) is a serious adverse effect associated with bisphosphonate therapy, especially in oncologic patients undergoing invasive dental procedures. Conventional treatments show limited predictability, which justifies the investigation of regenerative strategies. Objective: Evaluate the clinical efficacy of leukocyte and platelet-rich fibrin (L-PRF) in promoting tissue repair during oral surgical procedures in patients using zoledronic acid, focusing on the prevention and treatment of MRONJ. Study Design: This was a case-control study involving 20 patients divided into three groups. The control group (n=7) underwent surgical procedures without L-PRF. The prevention group (n=8) received L-PRF at the time of dental extraction to avoid MRONJ development. The treatment group (n=5) had existing MRONJ lesions and received L-PRF as part of surgical management. Patients were monitored for healing progression, pain, edema, and signs of infection. Results: The prevention group showed 100% successful healing, the treatment group had 80% success, and the control group had 57%. Use of L-PRF significantly reduced inflammation, postoperative pain, and infection rates. Conclusion: L-PRF demonstrated positive clinical outcomes in both prevention and treatment of MRONJ, serving as a promising autologous biomaterial to enhance healing and minimize complications in oral surgery for patients under bisphosphonate therapy.
