Resumo: Objective: To analyze the clinical and histopathological characteristics, as well as the prognostic factors of gingival squamous cell carcinoma (GSCC). Study Design: Case reports and case series of GSCC with histopathological confirmation were searched in PubMed, Embase, Scopus, Web of Science, and gray literature. Results: A total of 90 studies (109 patients) were identified. There was a slight predilection for females (n=57; 52.3%), and the lesions mainly affected the posterior lower gingiva (n=43; 39.4%). In the log-rank test, a significant increase in overall survival was observed in patients treated surgically (82.2%; p=0.004) and in individuals without local recurrence (80.9%; p<0.001). Kaplan-Meier curves estimated the 5-year overall survival at 62.4% and disease-free survival at 75.5%. In the univariate Cox regression model, individuals who underwent surgical treatment (hazard ratio [HR]=5.48; p=0.11) and patients without local recurrence (HR=6.96; p=0.01) showed better survival rates. Conclusion: Although GSCC presents a good prognosis, the tumor has a high recurrence rate within the first five years.
