Título: TIMING OF RECURRENCE AND PREDICTORS OF SURVIVAL IN ADENOID CYSTIC CARCINOMA: A SYSTEMATIC REVIEW AND META-ANALYSIS

Resumo: Objective: This systematic review aimed to evaluate survival outcomes in adenoid cystic carcinoma (ACC) by recurrence time and prognostic indicators. Study Design: A systematic search was conducted on March 25, 2024, across major databases for studies on ACC survival in head and neck sites. Risk of bias was evaluated with the Joanna Briggs tool, and meta-analysis was performed using R. Results: From 53 eligible studies, ACC predominantly affected women (mean age: 52.7) and commonly arose in minor salivary glands, often as a painful mass. Local recurrence occurred first (27.5 months; 19.7%), followed by locoregional recurrence (31.3 months; 28%). The most frequent and late outcome was distant metastasis, identified in 46.83% of patients, with an average of 34.8 months, and was also the main cause associated with mortality. Multiple combined outcomes were observed throughout the follow-up in a smaller proportion of cases. Overall survival declined from 73.8% at 5 years to 26.5% at 20 years. Meta-analysis revealed advanced T-stage, nodal involvement, solid histology, distant spread, and positive margins as poor prognosis factors. Conclusion: Distant metastasis is the most common and significant adverse event in ACC and may present during long-term follow-up. Prognosis worsens with advanced tumor stage and incomplete surgical resection.
