Resumo: Melanotic neuroectodermal tumor of infancy (MNTI) is a rare benign neoplasm that usually affects infants under one year of age, with a predilection for the maxilla. Thought to originate from neural crest cells, MNTI often presents asymptomatically. Diagnosis is supported by elevated urinary vanillylmandelic acid and catecholamines. Despite its benign classification, MNTI demonstrates aggressive and infiltrative behavior. Clinically, it appears as a brownish mass with irregular borders, elastic consistency, and may involve early tooth eruption or displacement. Histologically, it shows a biphasic pattern with melanotic epithelioid and undifferentiated small cells. This report presents a 10-month-old patient with maxillary swelling, diagnosed via clinical and imaging examinations, and treated with incisional biopsy, surgical excision, osteotomy, and cryotherapy. Described by Krompecher in 1918, MNTI remains a diagnostic challenge due to its rarity and ability to mimic other lesions. The patient continues under follow-up with no current complaints.
