Resumo: A 32-year-old female patient presented with a multilocular area of reduced bone density, with irregular borders and a ground-glass appearance in the ascending ramus of the right mandible, identified by panoramic radiography. An incisional biopsy was performed, and the specimen sent for histopathological analysis. Microscopy revealed round to oval cells with foamy, granular eosinophilic cytoplasm and small hyperchromatic nuclei within a fibrous stroma. The cells were immunopositive for CD68 and negative for S-100. The patient had no endocrine or metabolic disorders, as confirmed by laboratory tests, indicating a primary intraosseous xanthoma (IX). Surgical curettage is the preferred treatment, with favorable outcomes and low recurrence. This rare condition may mimic other jaw lesions, making histopathological analysis and clinical investigation essential for accurate diagnosis.
