Resumo: The diagnosis of serious systemic illnesses following the observation of oral manifestations should be a primary concern for dental professionals. We report the case of a 77-year-old female who presented with the complain of dental malocclusion after the use of mandibular advancement device to treat obstructive sleep apnea. This was followed by the development of a chronic ulcer in the border of the tongue. An incisional biopsy revealed chronic unspecific ulcer. This was followed by a diffuse enlargement and induration of the tongue leading to speech misarticulation. Another biopsy revealed deposits of pale eosinophilic amorphous extracellular material, stained by Congo red with apple-green birefringence upon polarization, suggestive of amyloidosis. The patient was referred to an oncohematologist service. Further investigation diagnosed amyloidosis due to kappa light chain multiple myeloma, treated with chemotherapy and bisphosphonates.
