Título: ANTIMICROBIAL PHOTODYNAMIC THERAPY IN ALVEOLAR REPAIR AFTER TOOTH EXTRACTION IN IRRADIATED HEAD AND NECK PATIENTS

Resumo: Objective: To compare antimicrobial photodynamic therapy (aPDT) with other forms of ORN prevention, evaluating the repair of the alveolar mucosa after extraction. Study Design: This was a non-randomized controlled clinical trial conducted in an oncology hospital. The control group consisted of antibiotic, pentoxifylline and tocopherol, and the interventions consisted of aPDT in one group and antibiotics plus photobiomodulation in the other. All medications were started before the surgical procedure and laser therapies were performed immediately after tooth extraction. The patients were evaluated in 4 moments, were considered bone exposure and time of total mucosal coverage of the alveolus. Results: 52 extractions were performed, of which 49 showed complete mucosal repair at the end of 28 days. Bone exposure was observed at five time points, but no cases of infection were observed. There was no statistically significant difference between the three groups in relation to mucosal repair. The cases were followed up for a mean period of 9 months and none progressed to ORN. Conclusion: aPDT allowed satisfactory mucosal repair in the time evaluated, and the present study points to a path that may favor, with additional studies, the establishment of the gold standard in this type of clinical approach.
