Resumo: A 47-year-old woman with a 16-year history of hypothyroidism and 10 years of endometrial polyposis presented with gingival bleeding and a painless, purplish nodule with yellowish areas on the alveolar ridge near teeth #28 and #29. The lesion, about 1 cm² with irregular borders, had been evolving for two months. The clinical hypothesis was pyogenic granuloma, though malignancy was considered. Incisional biopsy confirmed pyogenic granuloma, showing angiogenic proliferation with intense mixed inflammatory infiltrate. Due to postoperative growth of the lesion, a cone beam computed tomography was performed, revealing a feeding vessel at the lesion site. Complete excision was done using an 808 nm diode laser (Soft Lase ProT Zap Laser) at 1.2 W in pulsed mode (0.05 ms), with a 400 µm fiber and 90 J/cm² fluence, with cauterization of the vessel performed during the procedure. A five-month follow-up showed no recurrence.
