Resumo: Accurate imaging interpretation is critical in the diagnostic workflow of jaw lesions, especially when radiographic features overlap with those of aggressive odontogenic tumors. This case report describes a 32-year-old male patient referred for evaluation of the right mandibular third molar region. Panoramic radiography and cone-beam computed tomography (CBCT) revealed a well-defined, multilocular, corticated, and hypodense lesion extending from the mandibular ramus to the first molar region. Buccolingual cortical expansion and inferior displacement of the mandibular canal were noted. The differential diagnosis included odontogenic keratocyst (OKC) and unicystic ameloblastoma. An incisional biopsy and histopathological exam confirmed the diagnosis of OKC. Surgical enucleation and extraction of the second and third molars were performed, followed by placement of a drain. Imaging follow-up demonstrated progressive bone healing. This case underscores the importance of correlating CBCT findings with histopathological analysis to avoid misdiagnosis and guide appropriate surgical planning in extensive jaw lesions.
