Resumo: Mandibular lesions associated with impacted third molars are common findings in dental imaging and require accurate assessment for proper diagnosis and management. This case report presents a 29-year-old female patient referred for evaluation of the left mandibular third molar region. Panoramic radiography and cone-beam computed tomography (CBCT) revealed a well-defined, unilocular, corticated, hypodense lesion associated with the crown of the impacted third molar. Additional findings included buccolingual cortical thinning, inferior displacement of the mandibular canal, and external root resorption of the adjacent second molar. The differential diagnosis included dentigerous cyst, odontogenic keratocyst, and unicystic ameloblastoma. Surgical enucleation was performed, along with extraction of the second and third molars, followed by placement of a bone graft. Histopathological analysis confirmed the diagnosis of a dentigerous cyst. One-year follow-up imaging showed satisfactory bone regeneration and graft integration. This case highlights the importance of CBCT in characterizing pericoronal lesions and guiding regenerative treatment planning.
