Resumo: Oral metastases from colorectal cancer are rare and have a poor prognosis. A 55-year-old woman was referred to the medical service to evaluate a painful lesion in the retromolar trigone. During the medical interview, the patient reported colonic adenocarcinoma (CA) with metastases in multiple organs. The extraoral examination showed a hardened enlargement on the left hemiface. Intraoral examination revealed an infiltrative ulcerative vegetative lesion in the left retromolar trigone. Computed tomography demonstrated an expansive lesion with a lytic component and cortical discontinuity in the left ascending mandibular ramus. Microscopic analysis showed a proliferative epithelial component composed of irregular tubular structures lined by columnar cells with variable pleomorphism. Immunohistochemical analysis showed positive staining for CK20 and CDX2, while CK7 was negative. The Ki-67 index was 90%. The diagnosis of metastatic CA to the oral cavity was confirmed.
