Resumo: A 38-year-old male patient presented with painful, superficial ulcers on the labial, buccal, and lingual mucosa, persisting for five days and accompanied by submandibular lymphadenopathy. The patient reported fever preceding the onset of lesions and no improvement with topical corticosteroids. Clinical findings were suggestive of primary herpetic gingivostomatitis. Serologic testing for herpes simplex virus (HSV) and cytomegalovirus revealed only HSV type I IgG positivity. Exfoliative cytology showed epithelial cells with morphological alterations, including multinucleation, consistent with HSV infection. The patient underwent three sessions of antimicrobial photodynamic therapy, resulting in complete healing and symptomatic relief. This case highlights that primary herpetic gingivostomatitis can occur in adulthood and may present atypically. Recognizing classic signs and avoiding corticosteroids in active herpetic lesions are essential, as such treatment may exacerbate symptoms. Exfoliative cytology can support the diagnosis, especially when serology is inconclusive.
