Título: EXPLORING MOLECULAR ALTERATIONS IN OSTEOSARCOMAS OF THE ORAL AND MAXILLOFACIAL REGION: A SCOPING REVIEW

Resumo: Objective: To explore and summarize the molecular alterations reported in osteosarcomas of the oral and maxillofacial region, aiming to better understand the genetic background of these tumors. Study Design: A scoping review was conducted in accordance with the PRISMA-ScR guidelines. Eligible publications included case reports, observational studies, and systematic reviews that investigated molecular changes in craniofacial osteosarcomas. A structured search was performed across four scientific databases. Results: Twenty studies reporting on 68 cases of maxillofacial osteosarcoma were included. The mean age of patients was 39.4 years, with a slight male predominance. The mandible was the most frequently involved site, and the chondroblastic variant was the most prevalent histological subtype. TP53 mutations were the most frequently observed, followed by alterations in MDM2, CDK4, and other oncogenes and tumor suppressor genes. Surgical resection was the primary treatment, occasionally combined with chemotherapy. Local recurrence occurred in 11.1% of cases, while 16% developed distant metastases. Overall, 69.7% of patients were alive at last follow-up. Conclusion: This review highlights key molecular findings in craniofacial osteosarcomas, underlining the relevance of genetic profiling in diagnosis, prognosis, and treatment planning. Broader studies are still needed to refine molecular-based therapeutic strategies.
