Título: CLINICOPATHOLOGIC ANALYSIS AND SURVIVAL OUTCOMES OF SARCOMAS IN THE ORAL AND MAXILLOFACIAL REGION: A SYSTEMATIC REVIEW

Resumo: Objective: To systematically synthesize clinical and pathological data on primary sarcomas occurring in the oral and maxillofacial region, emphasizing patient demographics, tumor behavior, treatment strategies, and survival outcomes. Study Design: A systematic review was performed following the PECOS model. Observational studies reporting on primary sarcomas in the specified anatomical region were selected through a comprehensive search across five electronic databases. Survival data were analyzed using Kaplan-Meier estimates and Cox proportional hazards models. Results: Thirty-four studies comprising a total of 650 patients were included. The average age was 35.1 years, with a slight male predominance. The mandible was the most commonly affected site, and osteosarcoma was the leading histologic type. Surgical resection was the main therapeutic approach. The estimated 5-year overall survival rate was 54.3%, and the disease-free survival rate was 60.4%. Prognosis was significantly influenced by age, tumor size (T classification), stage grouping, margin status, and occurrence of local or distant recurrence (p < 0.05). Conclusion: Oral and maxillofacial sarcomas remain rare but aggressive tumors. This review highlights key clinicopathologic features and prognostic indicators, contributing to more effective diagnostic and therapeutic strategies for improved patient outcomes.
