Resumo: Primary Intraosseous Carcinoma (PIOC) is a rare malignant neoplasm derived from odontogenic epithelium, typically of cystic origin, and more common in men over 50. A 35-year-old man, non-smoker and non-drinker, presented with facial discomfort. Clinical examination revealed swelling in the right mandibular angle. Panoramic radiography showed a well-defined unilocular radiolucent lesion partially surrounding the crown of a partially impacted tooth #48. No mucosal changes or discharge were observed. An inflamed paradental cyst was suspected, and tooth #48 was extracted, followed by curettage. Histopathological analysis revealed a moderately differentiated squamous cell carcinoma. PET/CT and videolaryngoscopy ruled out other primary sites and confirmed the diagnosis of PIOC, likely associated with a paradental cyst. The patient underwent segmental mandibulectomy with cervical dissection, followed by chemotherapy and adjuvant radiotherapy. He remains under close follow-up with no signs of recurrence after five months.
