Resumo: Peripheral ossifying fibroma (POF) is a benign gingival lesion that can cause aesthetic and functional deformities when large. A male patient with a nodular lesion on the left mandibular ridge, with a reddish-ulcer color, irregular surface, firm consistency, pedicled and fixed, with an evolution of approximately five years. Clinically, there was marked tooth mobility, periodontal involvement and biofilm accumulation. Radiographically, it showed radiopaque areas surrounded by a radiolucent halo, suggesting calcifications in fibrous tissue. The patient had previously been referred to oncology centers and did not receive adequate care, which favored the growth of the lesion. Given the hypothesis of FOP, the lesion was completely excised and the local irritants removed. The anatomopathological analysis confirmed the diagnosis and no further intervention was necessary. The procedure was effective and immediately resolved the problem, restoring the patients function and aesthetics.
