Resumo: A 45-year-old patient undergoing treatment for multiple myeloma with bortezomib and zoledronate reported a "lesion on the roof of the mouth" during a dental appointment. Clinical examination revealed no visible intraoral alterations. Given the patient's history of biphosphonate use, a panoramic radiograph was obtained, followed by cone-beam computed tomography, which revealed an osteolytic lesion in the mandibular condyle, raising suspicion of oral involvement of multiple myeloma. Bone scintigraphy showed no increased uptake in the affected area. Based on a comparison with previous imaging, incisional biopsy was not indicated. The patient was referred to his oncohematologist and is currently under multidisciplinary follow-up. Periodic radiographic assessments are being conducted to monitor potential complications, including medication-related osteonecrosis of the jaw. This case underscores the importance of dental evaluations in patients with multiple myeloma, even in the absence of clinical symptoms, and reinforces the dentist's role in early detection and collaborative management with other healthcare professionals.
