Título: ORAL LEUKOPLAKIA TREATMENTS AND ORAL CANCER PREVENTION: A SYSTEMATIC REVIEW AND META-ANALYSIS

Resumo: Objective: To evaluate the effectiveness of different treatment modalities for oral leukoplakia in preventing oral cancer.
Study Design: A systematic review and meta-analysis of randomized controlled trials (RCTs) was conducted. Comprehensive electronic searches were performed in PubMed/MEDLINE, EMBASE, LILACS, Web of Science, Scopus, LIVIVO, and grey literature sources (Google Scholar and ProQuest) for studies published up to May 2024. Eligible studies included RCTs comparing any treatment for oral leukoplakia with outcomes related to cancer prevention.
Results: A total of 3,188 records were identified. After applying the eligibility criteria, 22 RCTs were included. The risk of bias assessment revealed 10 studies with a high risk, 7 with a moderate risk, and 5 with a low risk. The treatments assessed included conventional surgery, laser surgery (YAG and CO₂), surgery with safety margins, photodynamic therapy, and vitamin A supplementation. A meta-analysis of three homogeneous studies demonstrated a potential benefit of vitamin A; however, the certainty of the evidence was low.
Conclusion: Surgical approaches and vitamin A supplementation appear to be effective in managing oral leukoplakia for cancer prevention. However, the low certainty of evidence concerning vitamin A highlights the necessity for additional high-quality randomized controlled trials.
