Resumo: Pigmented oral lesions presents a wide range of etiologies, from benign exogenous deposits to potentially life-threatening malignancies. The clinical similarity between focal argyrosis and melanoma may lead to misdiagnosis, with significant clinical implications. This case-based review presents four patients (aged 59+), all with asymtomatic pigmented lesions (0.5-2.2 mm) in edentulous alveolar ridges. Periapicals revealed radiopaque fragments suggestive of amalgam. In one of them, CBCT for implant planning confirmed proximity to the mandibular canal. Diagnosis in these three cases was based on radiographic and clinical features, with conservative follow-up. The fourth case showed poorly defined borders and lacked raiographic correlation. Due to its resembalnce with melanoma and other pigmented lesions, an excisional biopsy was performed, confirming focal argyrosis withoutmalignancy. Accurate diagnosis is essential, combining clinical assessment with imaging allows for safer, more effective management of pigmented oral lesions and helps preventovertreatment or delayed intervention in cases of malignancy.
