Resumo: Cone Beam Computed Tomography (CBCT) has proven to be an essential tool in diagnosing craniofacial bone changes, especially in rare genetic disorders such as McCune-Albright Syndrome (MAS). This condition, associated with a GNAS gene mutation, presents varied clinical features, including facial asymmetry and polyostotic fibrous dysplasia. This report presents the case of an 18-year-old patient with left-sided craniofacial asymmetry and skin hyperpigmentations suggestive of MAS. CBCT revealed mixed hyperdense areas with a ground-glass appearance in the facial bones and cranial vault, which supported the diagnostic suspicion of fibrous dysplasia. Histopathological examination confirmed the condition, showing hypercellular fibrous connective tissue and vital bone trabeculae. Based on the integration of clinical, radiographic, and histological findings, the diagnosis of MAS was established. This case highlights the vital role of CBCT as the imaging method of choice for assessing bone structural changes in craniofacial syndromes, playing a key role in guiding therapeutic planning.
