Resumo: This report addresses a clinical case of a lingual ulcer in a 72-year-old retired male patient who reported 20 days of pain while eating and swallowing due to a "tongue sore" near a fractured tooth. His medical history was positive for hypertension, diabetes, hypercholesterolemia, gastric and neurological issues (stroke 23 years ago). He reported being a former smoker and alcoholic. Clinical examination identified a palpable, tender lymph node in the right infra-auricular region. Intraoral examination revealed a painful, hardened, raised-edge ulcer with a yellowish base, measuring 2 centimeters at its largest diameter, located on the right tongue margin. Diagnostic hypotheses included squamous cell carcinoma with lymph node metastasis and nonspecific ulcer. Symptomatic treatment for two days showed no improvement. An incisional biopsy under local anesthesia revealed "nonspecific chronic ulcer." Healing was satisfactory, and the lesion resolved within a month. The patient remains under follow-up care.
