Resumo: Pemphigus vulgaris (PV) is an immune-mediated mucocutaneous disease that can affect oral mucosa. A 66-year-old female patient positive to diabetes, hypothyroidism, and gastroesophageal reflux, reported stress, "recurrent aphthous stomatitis," dry mouth, halitosis, pain, and difficulty chewing. Clinical examination showed lip dryness, bleeding fissures, crusts, ulcerated and painful mucosal lesions, and ruptured vesicles. Nikolskys test was positive, suggesting PV or another vesiculobullous disease. Topical antifungal and symptom relief medications were prescribed. After laboratory tests and an incisional biopsy, PVs was confirmed. Treatment with systemic corticosteroids was initiated, leading to significant improvement within days. A G6PD enzyme test was requested for possible dapsone administration as part of the treatment. The patient is under outpatient follow-up. Accurate diagnosis of oral PV is emphasized for effective treatment planning.
