Título: ORAL SQUAMOUS CELL CARCINOMA 11 YEARS AFTER ALLOGENEIC HEMATOPOIETIC CELL TRANSPLANTATION WITHOUT cGVHD ACTIVITY

Resumo: A 28-year-old female patient, non-smoker, non-drinker, submitted to identical related allogeneic hematopoietic cell transplant for acute lymphoblastic leukemia. On D+180, she developed severe chronic graft-versus-host disease (cGVHD), affecting the skin, mouth, eyes, and liver, which was treated with topical and systemic corticosteroids and sirolimus (initiated on D+265). Immunosuppression was discontinued in the second-year post-transplant and there was no signal of cGVHD activity since that. Eleven years post-transplant, an ulcerated, painless lesion with a fibrinous pseudomembrane, measuring 1.5 cm, appeared on the right dorsum of the tongue. The incisional biopsy revealed well-differentiated, superficially invasive squamous cell carcinoma with intraepithelial dysplasia. Immunohistochemistry showed Ki-67 >70% and negative results for p16 and HSV1/2. The patient underwent partial glossectomy with clear margins and no cervical lymph node involvement (pT1pN0). Six months post-surgery, there has been no recurrence. This case emphasizes the importance of long-term oral monitoring in transplant recipients, even without cGVHD active signal.
