Título: ODONTOGENIC KERATOCYST MIMICKING THE PERIAPICAL CYST IN THE ANTERIOR MAXILLA

Resumo: A 54-year-old male presented with swelling in the anterior vestibular region. Clinical examination revealed a sessile, normochromic nodule extending across the midline, associated with the apices of non-vital maxillary central incisors. Cone-beam computed tomography demonstrated a unilocular, hypodense lesion with disruption of lamina dura, apical root resorption, and communication with the nasopalatine duct. Initial management included endodontic treatment of the central incisors; however, the lesion persisted. Under the differential diagnosis of radicular cyst and nasopalatine duct cyst, an excisional biopsy and apicectomy were performed. Histopathological examination revealed a cystic capsule containing satellite cysts, lined by parakeratinized stratified squamous epithelium with a palisaded basal layer and surface corrugation. These features confirmed the diagnosis of an odontogenic keratocyst. The patient remains under regular follow-up, with no signs of recurrence to date. This case highlights the importance of integrating clinical, radiographic, and histopathological findings for accurate diagnosis and appropriate management of jaw lesions.
